About IVF (In Vitro Fertilization)

IVF is an abbreviation for In Vitro Fertilization. Today, this process is has become a reliable option to support in the beginning. The In Vitro Fertilization includes the process of fertilizing an egg outside the body. This process can be performed only after the partners are confirmed as right applicants. The treatment is performed on the women only when she has been identified with endometriosis. This antibody difficulty would harm the egg or sperm in their body, so the egg must be fertilized outside the body with the help of In Vitro Fertilization (IVF). Many other problems could lead to using IVF such as problems related to Fallopian tubes, uterus or ovulation. The women who are willing to conceive a baby at the age of 35 and men having a lesser sperm count are also applicants appropriate for the treatment.

IVF procedure: Learn how IVF works

IVF is also a key for various fertility issues, poor sperm count issue, blockage in Fallopian tubes and immunological fertility. The IVF process would depend upon the fertility issue. The sperm and eggs can be of the partner’s own or can be given by the other donors. For the success of the IVF process, some of the treatments are:

  • Cryopreserved Embryo Transfer: In this kind of treatment process, the embryo would be frozen at a very low temperature which is then defrosted and transferred into the woman’s uterus.
  • Vitrification: This process includes freezing of the eggs, sperm and embryo which enable a later pregnancy.
  • Laser aided Embryo hatching: The laser helps in cutting out the embryo. This helps in breaking its shell and thinning the outer coating. This would enable the embryo to implant itself in the woman’s uterus.
  • Ovulation Induction: Some medications are used in this process to stimulate the ovulation in one or more follicles of the ovary.

What are the risks of IVF?

Some particular steps which are includes risk in the process of IVF are as follows:

  • Miscarriage: The rate of miscarriage is same for both methods for conceiving a baby. Having a baby normally or through IVF process, both of them carry the same amount of miscarriage risk. But the usage of frozen embryo in the process of IVF, may include some amount of miscarriage risks.
  • Multiple Births: If more than one embryo are implanted in a women’s uterus, there is an increased risk of having multiple babies. The multiple birth can lead to early labor, and the baby would have a low birth weight. If single foetusis implanted, then the risk of having multiple birth would be very low.
  • Ectopic Pregnancy: The people conceiving a baby using the IVF procedure will have an Ectopic Pregnancy. This happens when the egg fertilized outside is implanted outside the uterus, generally in a Fallopian tube. The egg cannot survive in this place and there would be no way to revive the pregnancy.
  • Ovarian Cancer: There has been some researches and studies which suggest that the process of IVF by certain medications can lead to ovarian tumour. Many other recent studies do not agree with these findings.

What is the success rate of IVF Process?

Every woman’s chance of conceiving a baby reduces with age. If the women are conceiving the birth with the help of IVF with her own eggs, then on an average the women with a young age has a higher success rate than a woman who is older.

What is the cost of IVF?

Generally, the IVF procedure costs around Rs60, 000 – Rs80, 000 in India. But some partners end up spending around Rs.3-5 Lakhs because they might need other treatments. Other treatment may be required depending on the medical history and complications in women’s fertility levels.

Step by Step IVF Procedure

The technique of performing the IVF many vary depending upon the individual’s circumstances of the clinic. Various consent forms are needed to be filled before the treatment starts. Some blood tests would be taken to see if any of the partner is having Hepatitis, HIV or Human T cell virus. After these formalities are done the treatment of IVF starts. This treatment involves the following steps:

For Women:

  • Destroying your natural monthly hormone cycle: In the first step the women would be given a drug to destroy the natural hormonal cycle. The women can control her cycle by a daily dose of Injection or spray. The treatment of drugs would continue for around 2 weeks.
  • Increasing the egg supply: After destroying totally the natural hormone cycle, gonadotrophin hormone drug would be given to the women. This drug treatment would continue for 12 days. This hormone would help to produce increased the egg count.
  • Inspection of Progress: Throughout the drug treatment the clinic doctors would monitor your progress with the help of vaginal ultrasound scans. Some blood tests are also taken to view the progress of the treatment. In order to make your eggs mature, the doctor would give the women another hormonal injection before 34- 40 hours of collecting the eggs. The hormone injection which would be injected is likely to be Human Chorionic Gonadotrophin (HCG).
  • Collection of the eggs: You would have sedated before collection of the eggs from the woman’s uterus. The eggs would be collected with the help of ultrasound guidance. The ultrasound probe would be attached to a hollow needle and is used to collect eggs from each follicle of the ovary. The women may experience some cramps and small amount of bleeding from her vagina. After the collection of your eggs, you would be given medication in the form of gel or injection. This would help prepare a lining for the transfer of the embryos in the uterus.
  • Fertilization of the eggs: The eggs collected from the women’s ovaries would be then mixed with the sperm of your partner or a chosen donor. The mixture is then cultured in the laboratory for around 15-20 hours. After which they check for the signs of fertilization. After the fertilization it has now become embryos. The embryos will then be grown in the incubator for 6 days. The embryologist will then monitor the progress of the embryos. The best embryo would be chosen by the embryologist for the transfer. The remaining embryos would be preserved by freezing them for the future use.
  • Transfer of the Embryo: The count of the embryo transfer would depend upon the age of the women. If the age of the women is more than 40, then a maximum of 3 embryos would be transferred. If less than 40, 1-2 embryos will be transferred. There is a restriction on the number of embryos to be transferred is because of the risk involved with multiple birth. The clinic would recommend the best option for the women’s health.

At the time of the process, the doctor will inject a speculum into the women’s vagina. The speculum is used to wide open the vagina so that the cervix could be visible. A thin pipe is then passed through the cervix using the ultrasound method. Then the pipe is passed into the womb of the women. For this process no sedation is required as it is a pain free procedure. A full bladder is required for using ultrasound, so a little discomfort can be experienced.

For Men

A sample of the man’s sperm would be asked while the eggs of your partner are collected. The sperm would then be properly diagnosed and be washed properly. Then the active and normal sperms are separated from poorer quality sperm. If the sperm has been stored before, then the sperm would be taken out of the frozen storage and then would be defrosted. Then it would be washed and prepared in the same way.

Success Rates

The success rate of having a baby with the help of IVF totally depends upon the age of the women. If the woman is above the age of 35, then the rate of success is very low. If the woman is young, then the possibility would be high for conceiving a baby using IVF. The success rate of IVF also depends on other factors such as quality of embryos and their preservation. The IVF and intra-cytoplasmic injection of sperm has the same success rate and are not presented as a separate process nowadays.

IVF success rates with fresh embryos

The IVF success rate tends to be higher or lower depending upon their age. The women have to go through a hormonal treatment to control her menstrual period and help the eggs to mature. When the eggs are matured, they are fertilized and harvested. The best fertilized embryos are then transferred to the uterus of the women. Fresh embryo cycles have been used for decades. In the fresh embryo method of IVF the women have to go through the ovarian stimulation. The procedure would involve the whole laboratory procedure for developing the embryos. After that the embryos would be chosen for either fresh or frozen processed of IVF.

The percentage of success for the women using fresh embryos, according to their ages is as follows:

  • 33% for women aged under 35.
  • 27% for women aged between 35-38.
  • 20% for women aged between 39-40.
  • 12% for women aged between 41-43.
  • 4% for women aged between 44-45.
  • 2% for women aged over 45.

IVF success rate with frozen embryos

The process of freezing and defrosting the embryos has been used for a long time. This process is preferred over the fresh embryo transfer procedure. This has a higher success rate and efficiency. This method uses vitrification for conceiving the birth. This method has led to a much better pregnancy rate. The fertility specialists are claiming that the frozen cycle would be better if the natural hormones are kept in a more environmental place in the uterus.

The percentage of success, having a baby with the frozen cycle method is as follows:

  • 40% for women aged under 35.
  • 36% for women aged in between 35-37.
  • 31% for women aged between 38-40.
  • 24% for women aged between 41-42.
  • 17% for women above 42.

Factors affecting the IVF success rate

There are some important factors which affect the probability of IVF treatment are as follows:

  • Age: Age has been an important factor which affects the success rate of IVF procedure. The women of ages over 35 would have a lower chance of conceiving a baby through IVF. Women under the age of 35 would have a higher chance of having a baby. The younger the women, the chance of them to have a baby are high. The relation between the age of a woman and the success rate is inversely proportional.
  • Fertility Issues: The problem of infertility or low fertility occurs in the women due to endometriosis or blockage in the Fallopian tube. The woman may also face some issues with ovulation. Male’s low sperm count is also an important factor affecting the success rate of conceiving a baby with IVF.
  • Previous Pregnancy: If the woman has already been pregnant before through a normal way or with some ART, then that woman would have a greater chance of getting pregnant again with the help of IVF. A woman having recurring miscarriages indicates a lower success rate. But if a woman hada miscarriage before, she must not opt out the option of getting pregnant with IVF. The doctors could narrow down the causes of miscarriage and recommend you the best option of having a baby.
  • DonorEggs: The male or female, who is older, should opt for a donor’s sperm. Because of the age factor the partner, the success rate would be very low for them. If they chose a donor for them, they would have a higher chance of having a baby.
  • Lifestyle: The success rate also would depend upon the lifestyle of the partners. If any of them has a habit of smoking, then this may lead to a major reproductive consequence. Another factor which affects the success rate of IVF is being overweight. Being overweight can have a strong effect of the IVF procedure.
